Use this form to register a new food premise, to renew an existing licence or to transfer a registration to another person.

Bunrunner certificate - application form (127k PDF) (1 page)
Use this form if you want to apply to sell food in streets or public places. A bun runner certificate will be issued only when the applicant operates from a registered food premises and meets any other conditions required by an Environmental Health Officer. Bun runner licences expire on 31 August each year.

Foodstall Health Certificate - application form (73k PDF) (1 page)
Use this form if you want to apply to operate a foodstall/mobile shop. Certificates are only valid for 12 months.

Health protection licence - application form (48k PDF) (2 pages)
Use this form if you are required to apply for a health protection licence. The business activities that require such a licence by law include but not limited to: Fitness and exercising programmes, electrolysis/epilation, red vein treatment, waxing, ear/nose piercing, acne treatment, manicures, hair implantation, pedicures, pigment implantation/tattooing, use of baths, spa pools, or swimming pools, use of sun beds, use of saunas, use of steam rooms, acupuncture/moxibustion, massaging, or other.

Use this form if you are applying for registration of a hairdressers shop. All certificates of registration for hairdressers shop expire on 31 July of each year.

Registration of offensive trades - application form and guide (31k PDF) (2 pages)
Use this form if you are applying for registration of an offensive trade. The types of offensive trades that require such a licence by law include but not limited to: Blood or offal treating, bone boiling or crushing, collection and storage of used bottles for sale, dag crushing, fell mongering, fish cleaning, fish curing, flax pulping, flock manufacturing or teasing of textile materials, gut scraping and treating, nightsoil collection and disposal, refuse collection and disposal, septic tanks desludging and disposal of sludge, slaughtering of animals for any purpose other than human consumption, storage, drying or preserving of bones, hides, hoofs, or skins, tallow melting, tanning, wood pulping, wool scouring, and other.

Liquor licensing

As part of the transition to the new Auckland Council, some regulatory forms are being standardised. Prior to 1 November 2010 these forms can be used at any local government organisation in the Auckland area.

UpLiquor licensing temporary authority - application form (217k PDF) (3 pages)
Use this form to apply for a temporary liquor licensing authority for any person who has any 'right title estate or interest' on a premise that already has an on or off license - as liquor licenses are not transferable, this enables the premise to continue operating while the new owner obtains his/her own licence.

Managers certificate application form (224k PDF) (5 pages)
Use this form to apply for a Managers certificate. At all times when liquor is being sold or supplied to the public on any licensed premises a manager must be on duty and responsible for the compliance with the Act and the conditions of the licence.
